They review all prescription orders and make recommendations for alternatives to prescribers. Consultant pharmacists also review clinical cases for polypharmacy (the simultaneous use of multiple drugs by a single patient, for one or more conditions), medication side-effects, and changes in conditions and make recommendations to the prescriber. 

Clinical Staff: Grane Rx interacts daily with the clinical staff at each PACE center to coordinate prescription orders from the prescribers through the electronic health record (EHR), administer medications to the seniors, and monitor for changes in conditions that may be related to the medication. The Grane Rx customer service team and pharmacists are available 24/7 in the pharmacy for direct contact with clinical staff for any questions or needs.  Clinical staff are the frontline of medication administration and patient care.
